Methods of S-line Rhinoplasty.
A. Indication for rhinoplasty surgery.
The appropriate patient for this procedure should have these characteristics:
- Low and short nasal bridge.
- Big nostrils

B. S-line Rhinoplasty by your own cartilage.
In this method the surgeon will harvest the patient cartilages from patient's rib, nasal septum, ear or temporal fascia to become the main material for reshaping the nasal tips, it could be used to combine with nostril reduction in the same procedure. + Surgiform, gore-tex and siligore.
Surgiform and gore-tex are both brands of the same material called expanded polytetrafluoroethylene (ePTFE), but they come from different suppliers.
Sili-gore implants is a specialized implants that use a thin gore-tex layer to cover its silicone core. This implant is appropriate on procedure for customers with very thin nasal skin.
Rhinoplasty process.
A beautiful nose has a length of the nose equal to 1/3 of the length of the face, the width of the alar base is equal to the distance between the eyes. The ideal nasolabial angle is between 95 to 105 degrees and the tip of the nose must be slightly higher by forming a small supra tip breaks that help define the nostrils shape, the mentioned characteristics combined with the appropriate shape of the (nasal dorsum) create this S shape when we observing the areas from the forehead to the nasal columella.

There are two techniques used in Rhinoplasty, which are closed and open rhinoplasty. Closed rhinoplasty can be used for nasal augmentation rhinoplasty while open rhinoplasty can be used for the structural rhinoplasty.
1. Closed rhinoplasty technique.
This procedure is performed under local anesthesia. The nose (nasal) augmentation procedure typically takes around 15 to 20 minutes to complete, though more complex cases may require additional time.
During the nasal augmentation procedure, the surgeon will make a small 1 cm incision inside the nose. The nasal skin is then detach from the bone and cartilage framework to from a small pocket, and an alloplastic implant is placed into the pocket to add the desired volume and projection. The natural appearance of the new nose will depend on your congenital nasal bridge shape, as well as the skill of the surgeon.
In the case of nose lift with cartilage covering the tip of the nose, the surgeon will take additional ear cartilage to wrap it around the alloplastic implant. This technique helps to avoid the common issues of redness or a point nasal tip with simple alloplastic implants in rhinoplasty.
Advantages: This procedure is simple, cheaper than structural rhinoplasty, and do not leave any visible scars.
Disadvantage: This method can only enhance the shape of the nasal dorsum. However, its depend a lots on the compatibility of the chosen synthetic material and the body, in some cases it can even causing implant exposure and visibility due to the thin skin. In order to avoid the mentioned problem and also achieving the ideals nostrils profile, cartilage wrapping is required.
2. Open Rhinoplasty Technique for Structural Rhinoplasty.
Structural Rhinoplasty refers to the open surgical technique that required the surgeons to make incision on the nasal columella to be able to lift the nasal tissues and make its underlying structure exposes or "open" visibly to the surgeon.
We can divide this method into two subcategory depends on the material used. While reshaping the 1/3 bottom parts of the nose (nostrils, alar base, nasal septum and columella) can only used autologous cartilage harvested from the ear, rib , and septal dorsum. The difference to distinguish the two options is based upon the material used to enhance the nasal dorsum. If the surgeon use the Alloplastic implants, it is called "Rhinoplasty with Cartilage and Alloplastic Materials", it will be referred as "Cartilage grafting rhinoplasty" if the surgeon used the autologous cartilage instead.
This method is applied in cases the nostrils shape is not ideal or when the nose having deformity that can not be simple fit by placing implant. This method is considered the most effective technique for achieving the perfect nose shape. The structural rhinoplasty procedure is performed under local anesthesia or general anesthesia.
Structural Rhinoplasty under Local Anesthesia or General Anesthesia.
This procedures typically takes between 60 to 90 minutes. The surgeon will make incisions through the columella (the skin between the nostrils), then harvest the patients cartilage (from the ear, rib, or septum) to reshape the tip of the nose.
In cases where the nasal dorsum is crooked or dorsal hump exist, the surgeon needs to perform an osteotomy to carved away the excess bones or cartilage, reshaping and smooth the underlying bones using bones rasp to achieve a more refined and slender nasal shape, then the surgeon harvest the required cartilage. Finally, synthetic Alloplastic implants will be placed to raised the nasal bridge up.
Advantages: Structural Rhinoplasty can help lengthen the nose while reduce any associated complication risks such as redness or implant exposures and extrusion.
Disadvantages: This method is considerable higher in price compare to the closed rhinoplasty method. In cases of using the alloplastic implants to enhance the dorsum bridges there is still remain a small risks of allergic due to patient incompatibility with the material.
Post-operative care for S-Line Rhinoplasty and Structural Rhinoplasty.
Bandage, steri-strip and nasal airway splint can be applied, this help keep the structure in place during healing period, creating a more beautiful and symmetrical appearance.
Just like any other surgical procedure you will experience swelling and soreness around the surgical sites, these sensation is especially strong in the first 24 hour after the procedure is finished. The patient can intake the prescribed pain relivers to help ease the healing process. They should strictly follow the post operative recovery guide such as rest in bed with your head elevated (except when you have to go to the bathroom) on the first day after S Line Rhinoplasty.
The area around the eyes will be swollen and bruised the next day. Applying an ice on the swollen areas for the first 2 days will help reduce the bruising and make you feel less uncomfortable.
While the above paragraphs provided expected healing symptoms, don't be too worried, our patients often reported experiencing lower pain level compares to what they have thought. Also the swelling, pain and bruising will gradually subside and fade away in about 5 to 10 days later.
What is rib cartilage rhinoplasty and its pro and cons?
Rib cartilage rhinoplasty refer to structural rhinoplasty that reshape or reconstruct the entire nose (nose bridge + tip + nose column) using primary autologous cartilage harvested from patient's rib (excess amount that can be harvested in other options).
First, the doctor will your harvest your rib cartilage and divide them into 2 portions, the first will be used to reshape the bottom third of the nose (nostrils, nasal columella and alar bases), often being used to lengthen the nose by raising the nasal tips. The remaining will be used to graft the implant that placed into the nasal bridge of the nose.
Advantages: This method is improved upon the Alloplastic and cartilage materials structural rhinoplasty by replacing the alloplastic materials with grafted implant using autologous cartilage, it remove any remaining risks of allergy with foreign material.
Disadvantages: The technique is complicated so the cost is much higher than structural rhinoplasty.
What is an attractive nose for men, and is rhinoplasty more difficult for men than for women?
While the surgery technique is the same, the male rhinoplasty tend to achieve different objective or changing the nose into a different profile:
- The male nose, when viewed from the side, tends to look straighter compared to that of women.
- When performing rhinoplasty on men, the ideal nasolabial angle is around 90 to 95 degrees (much smaller than woman's cases).
- A man's nose should appear longer and larger to emphasize masculinity.
- The nose and nasal tip of men are generally broader than those of women.
