I. What is DHI (Direct Hair implantation) technique?.
Direct hair implantation (DHI) is a technique for hair graft implantation, which refer to the steps of implanting harvested hair follicles into the recipient areas, the hair follicles is collected using either FUE or FUT methods. In the DHI method, the surgeons uses either traditional implantation pen or CHOI pen to put the hair follicles into its designated position. Using the CHOI pen, the doctor can control characteristics such as angle, growth direction and depth of implanted follicles. However, this method do not need the surgeon to use Sapphire scalpel to making the implantation site like the traditional methods.

II. Characteristics of combining DHI and FUE for hair transplant.
While DHI is less invasive implantation method compare to traditional one that lead to intervention reduction in recipient sites. FUE is a new hair follicles (grafts) harvesting methods that aim to reduce surgery time and remove the need for large incision in the donor areas. Combining them into one treatment session is the optimized method that can both reduce procedures time, complication risks and increase the success rate of the hair transplant procedures. Here is a few key information to keep in mind about this treatment combination.
- Mini punch motor or hollow punch scalpel will be used to harvest hair follicles.
- The harvested hair follicles will be preserved using specialized solution.
- We will implant the grafts into the recipient sites directly (We use CHOI pen at Viet My Plastic Surgery Hospital)
- We can implant up to 60 graft per square centimeters using this method.
- In each treatment session, we can harvest up to 4000 graft by using FUE method.
- There is no need to shave hair at the recipient sites, but it is compulsory to shave hair at the donor sites
III. DHI treatment details (step by step guide)?.
1. Consulting.
The trichologist determines the characteristics of each patient during the consultation by considering the cause of hair loss, the pattern of hair loss, the likelihood of hair loss progressing in the coming years, hair type (thick or thin), the patient's expectations, age, gender, and medical history.
During the consultation, the hairline will be drawn based on the patient’s natural and desired hairline. The recipient areas are determined by the number of grafts needed in each area and the number of grafts that can be harvested from the donor site. Patients may or may not be eligible for DHI depending on their degree of hair loss.
2. Preparation.
Preoperative tests for HIV, Hepatitis B and Hepatitis C are done at this stage. The patient’s hair is then shaved. However, if the patient requires fewer grafts, shaving is not necessary. Once sterilization is complete, the patient is ready for local anesthesia.
3. Local anesthesia.
The surgeon administers local anesthesia into donor areas before harvesting hair follicles.
4. Hair follicle harvesting.
Hair follicles are extracted from donor areas using a micro punch motor.
5. Load the hair follicles into the pen.
The grafts will be taken from preservation solutions, which help increase its survival rates.
The hair follicles are placed one by one in the CHOI hair transplant pen. The hair follicles are placed inside the needle tips for direct hair implantation instead of using forceps like traditional methods.
6. Hair transplant into the scalp.
In DHI, hair follicles are directly implanted into the recipient area using a Choi implant pen with a diameter of 1 mm or less without any prior skin incision. Therefore, there is no need for an additional “recipient site making” step; the implantation and hairline incision are performed simultaneously using the needle tip in the CHOI implant pen. This results in shorter surgery time and less pain.
